Frequently Asked Questions About Silverstone Memory Care of Appleton 

We offer three monthly rates that accommodate residents who have care needs ranging from independent but requiring cuing and supervision to full hands-on care including use of equipment such an EZ stand or hoyer lift. A formal health assessment is completed prior to admission and is used to determine the appropriate rate level.

Each rate is a flat monthly charge and includes a private room and bathroom. The rate includes all care and services such as meals, medication administration, and activities. There are no additional “care level” or extra charges added onto a base fee.

Additional costs above the monthly rate may include cost of medications from the pharmacy, beautician services, transportation, cable TV and phone in resident room.

Invoices are sent out monthly and payment is on a month to month basis. Prorated admission rates and refunds are made if applicable.

We accept private pay funds and public funding through the Family Care program.

Rates are increased annually to offset cost of living wage increases for staff, increases in food and supply costs and other basic operation costs such as utilities.

In addition, a resident’s rate may change at any time due to a significant change in care needs.

All rate increases are preceded by a 30-day written advance notice.

We look to family members to assist with taking their loved one to medical appointments, however if that is not possible, our staff will always arrange for and accompany a resident to an appointment, including to urgent care or the emergency department. A resident will never be sent to an appointment alone.

We are experienced in caring for residents ranging from minimal to extensive care needs and are dedicated to caring for residents through end of life. We have a strong collaboration with Hospice.

Our licensing requirements do not allow for ongoing “skilled” nursing care. Therefore, this may be a reason for discharge depending on the individual circumstance.

We are required to following CDC, DHS and local public health authority guidelines in order to protect our residents and staff. Window visits are allowed and during cold weather, visits may be done in the entry foyer between the glass doors using our intercom system for communication. We make every effort to keep residents connected to their families using all available technology including Facetime, email, social media updates, etc.

“Compassionate care” in-person visits for residents who are actively dying are allowed with certain restrictions, including the use of full PPE for visitor(s), limitations on number of visitors and length of visit inside of the building.
